The incident occurred when the rocket's upper stage disintegrated over the Caribbean, prompting the FAA to activate a Debris Response Area to manage aircraft traffic in the vicinity. No public injuries were reported, but the FAA is collaborating with SpaceX and local authorities to verify claims of property damage on the Turks and Caicos Islands. SpaceX stated that a fire in the aft section of the ship led to a 'rapid unscheduled disassembly.' Elon Musk commented that a preliminary indication suggests an oxygen/fuel leak was the cause.
