An eight-month inquiry into a deadly fire in Johannesburg, South Africa, which occurred last August and resulted in the deaths of 77 people, has concluded that city authorities and their entities were responsible due to years of neglect. Justice Sisi Khampepe, who led the inquiry, reported that officials ignored multiple warnings and allowed the Usindiso Building to fall into disrepair, which directly contributed to the tragedy and exposed a housing crisis. The findings were handed over to Gauteng Premier Panyaza Lesufi.
